MDA Patient Referrals
For Physicians
Physicians or their staff may contact the Clinic directly to refer patients with a neuromuscular disease.

For Patients
Neuromuscular patients may come to the MDA Clinic to confirm a diagnosis, to recieve supportive care, and to obtain genetic counseling. Call the Muscular Dystrophy Association (see contact info below) if you:
- are transferring from another MDA clinic
- are seeking supportive care for a confirmed neuromuscular diagnosis
- are seeking confirmation for a diagnosis of a neuromuscular disease
- have a family history of neuromuscular disease and wish to receive genetic counseling or genetic testing
The following process is recommended for individuals who think they may have a neuromuscular disease but have not yet been diagnosed:
- See a primary care physician for evaluation
- Obtain a referral to a neurologist
- Receive a confirmed or a probable diagnosis of a neuromuscular disease from a neurologist
Registration with the Arizona MDA Program
After the referral process is complete (as described above), call to register with the Arizona MDA program.
