Create Your Own Patient Resource Guide

Your Patient Resource Guide is designed to help you take an active role in your care:

  • keep your medical information organized in one centralized location
  • communicate your health information to your healthcare providers clearly, efficiently, and accurately
  • make the most of your doctor's appointments

How to create your guide u My Patient Resource Guide

Filling out your Patient Resource Guide may take some time initially, but it will save you time in the future because you will have all of your medical information in one centralized location. If you do not have all of your health information, just start with what you have. Take your notebook with you to all appointments, procedures and hospital visits.

The more information you can provide for your doctors during your appointment, the better your doctors can understand your healthcare needs and the more information they can usually give you.

Contact Patients

Family and friends may call the hospital at 602-406-3000 anytime to reach a patient who has been admitted.

The operator will connect callers directly to the patient's room provided they are accepting calls.

CarePages

Patients have the opportunity to contact friends and family by phone from their room or via CarePages.

CarePages are free, personal, private, secure web pages available on the St. Joseph's website that help family and friends communicate when someone is receiving care in our hospital. This free, easy-to-use service enables concerned family members and friends to post and receive news updates and messages about a patient.

Internet Access

Several locations in the hospital also have wireless Internet access for family members and visitors to use, but certain restrictions may apply.
